Usually the DM will send you the preferred form if you're eligible for one from the plot, when it comes time to request items. I know I've had several variations throughout my DMing tenure because I wanted to ask different questions in different plots.
If you haven't gotten one for a plot you're in and are waiting on it, ask the DM to make sure you weren't forgotten and items are being handed out!
