If your using steam, did you put the new portrait files here;  Steam\steamapps\common\Neverwinter Nights\data\prt

And did you turn it into an appropriate portrait format (.tga)? There should be 5 files for the 5 sizes used, Huge, Large, Medium, Small and Tiny. If not, you can turn an image into a portrait using the link below.
