One good round of tuberculosis can give anyone a reason to need to retrain. another option is to simply come out of retirement. The easy life takes its toll on someone.
Neither of those explain a loss of divine power, though.

Tangentially related, but what book are the monk orders from? I can't find anything on Old Order or Serpent Guards online, and I need to research them for reasons.
Well what you can find online is basically all that's mentioned anywhere for the Old Order. There isn't a lot on them in general. All that's to be found on the Serpent Guards is a blurb from the Faiths and Pantheons web enhancement, found here:

"These fanatics guard temples, serve as minions to
 dragons, hunt dragonslayers, and assassinate those who
 pry too closely into the activities of the church. They are
 active in Unther and have been known to attack Mulhorandi soldiers."

If you're going pure monk, then you begin with it, since you start at level 3, and the feat is given at 3rd level.

We tried to set it so that it addresses the worst problems while not affecting the bulk of players.

As for any bizaare edge cases, we can always leto the feat onto you early.

The particular edge case that comes to mind is an ECL race, which starts at 2.  But since those require LETOs anyway, I suppose that is a viable and not particularly obstructive solutioj.

Also, meeting a Quell can cut the divine from a priest or priestess or Paladin.  However, meeting one is rare.

Quells can also only affect a divine spellcaster for a period of up to 24 hours. The Quell's Intercession ability functions as a Turning check. As a normal hit, it cuts a cleric off for only a minute. At double HD, a day. In either case, it's never permanent and can be fixed even so by an Atonement spell.

There are however other curses that CAN cut a divine caster off from their deity. 

There's also, as noted, penalties for changing deities, leaving orders, etc. 

The atonement spell in and of itself also can cost quite a bit of XP for the caster (about 500 xp per sin), and whatever caster you got to do it decided to make you pay it.

I'm currently building for a halfling Rogue/SD/Invisiblade. She'll be using throwing knives as her ranged option because of no bow proficiency besides the equally questionable crossbow.
Xbow+ rogue+ rapid reload = awesomeness.

This is certainly true, although, the build I mentioned leaves me pretty damned feat starved as is. Using darts is not without its advantages over bows, especially when used by an Invisible Blade, allowing them to retain their AC bonus and also activating their Wounding proc. That's not to say bows wouldn't be superior, but throwing isn't without its merits.
